Palliative and End of Life Care at Home

We provide compassionate palliative and end of life care at home, working closely with local hospices, hospitals and clinical teams to support people whose preferred place of care and death is at home. We accept Fast Track referrals for clients with a short prognosis (typically less than three months) and focus on comfort, dignity and personalised support for the person and their family.

Practical support and equipment

We organise and support use of clinical equipment at home, including oxygen concentrators, suction equipment, pressure-relieving mattresses and feeding pumps, and liaise with suppliers and clinicians to ensure safe installation and training.

Family support and communication

We provide clear daily updates, involve families in care decisions and offer sensitive guidance on what to expect. Our staff support relatives with practical tasks, emotional presence and signposting to specialist bereavement and counselling services.
